Pronto is a crime novel written by Elmore Leonard and published in 1993. Leonard introduces three main characters and gets them moving against each other. Harry is constantly reminiscing about World War II. Tommy carries a picture of the old crime boss Frank Costello in his wallet. Raylan is a U.S. Marshal who wears a cowboy hat. In addition, the inclusion of the Ezra Pound stories add more to the understanding of Harry and his reasons for retiring to Rapallo, Italy.
Pronto: A Novel
It was followed by the 1995 novel Riding the Rap. A 1997 made-for-TV film version of Pronto starred Peter Falk, Glenne Headly and James LeGros as Arno, Patton and Givens, respectively. The character of Raylan Givens would later become the protagonist for the television series Justified.
The role of inherited thrombophilias in the pathogenesis of thrombosis, and their relation to thromboembolic events and pregnancy complications, has escalated the demand for molecular testing. We employed the new Pronto ThromboRisk kit (Pronto Diagnostics Ltd., Rehovot, Israel), which is based on a novel primer-extension ELISA assay, for the simultaneous detection of three genetic mutations. These are Factor V Leiden G1691A (R506Q), prothrombin G20210A, and methylenetetrahydrofolate reductase (MTHFR) C677T, which are known to have increased incidence in patients with thromboembolic events and pregnancy complications. We examined 284 randomly selected patient samples from the special coagulation hospital laboratory. The results using the new assay were compared to those obtained by routinely employed conventional molecular techniques. The ThromboRisk kit provided identical results with no false-negative results and with a high specificity of over 99% for all three mutations. We conclude that the Pronto ThromboRisk kit allows fast, precise, and reliable testing for the three genetic mutations. The assay is easy to perform and provides a useful tool for screening high risk populations, such as patients with a personal or family history of venous thromboembolism, women with pregnancy complications, or users of oral contraceptives.
Stent thrombosis is a rare but catastrophic complication of percutaneous coronary intervention (PCI). Thrombolytic therapy is ineffective for the treatment of patients with stent thrombosis, while primary PCI in such patients is limited by a high thrombus burden in the culprit vessel often leading to distal embolization and extensive tissue infarction. We present three patients with stent thrombosis successfully treated with the adjunctive use of a novel and new aspiration thrombectomy device (Pronto).
The Impella recover LP 2.5 is a percutaneous left ventricular assist device (LVAD) recently approved for use in patients undergoing high risk percutaneous coronary intervention (PCI) and also in cases of cardiogenic shock. There is limited evidence available in literature about its safety, especially with regards to the incidence of local vascular complications, their management and long-term implications. We report here the first case of a serious local vascular complication-superficial femoral artery thrombus formation during Impella recover LP 2.5 use in a high risk PCI which was managed successfully with novel aspiration thrombectomy catheter (Pronto V3), which in itself is the first reported use of Pronto V3 in such a vascular complication.
The Justified series became an instant hit, and prompted author Elmore Leonard to undertake a novelization project. He has since published a novel titled Ryalan (2012) from the Justified TV series produced by FX Network.

Standard modality for crossing a stenotic lesion is using a wire to cross the lesion in a native or graft vessel. The area of interest beyond the lesion will generally be visualized by angiogram. However, in certain cases with acute to sub-acute occlusion with limited TIMI flow one may have difficulty in locating the site of stenosis. This may occur for example when there is a distal stenosis of a bypass graft or superimposed thrombus throughout a vessel. These types of lesions may lead to difficult decision making in regards to plan of treatment, and tend to lead to increased risk of complications and unsuccessful outcomes. Dissemination of simple safer technique will help to reduce complications during these types of procedures. We describe a novel technique of visualizing and locating such areas of stenosis demonstrated via two cases; one involving native and one involving graft vessels.
We offer a novel technique in determining the area of stenosis during coronary angiography. Our technique as described above uses an over the wire balloon. The balloon is inflated until it is occluding the vessel which is followed by contrast injection through the proximal port of the catheters lumen. By doing this, opacification of the area just distal to the balloon is now able to be visualized. The balloon is then deflated, withdrawn backward two to three centimeters and the above process is repeated. In doing this, one may be able to visualize five or more centimeter segments of the distal vessel at a time depending on the vessels caliber.
